A vehicle service request form is the structured document a customer fills out to schedule service, diagnostics, or repair work with an auto shop, dealership service center, or mobile mechanic. It captures the customer's contact info, the vehicle's year, make, model, VIN, and mileage, the service the customer needs, a description of the issue, and the customer's preferred drop-off date. Each field was chosen to serve a specific operational or technical purpose. Here's what each one does — and why it's there.

Full Name

The customer's name as it appears on their driver's license and vehicle registration. Used to match work orders, invoices, and service history.

Email Address

Primary contact for the service confirmation, repair estimate, and invoice. Also used for post-service follow-up and review requests.

Phone Number

The preferred channel for same-day updates when the tech finds additional work or when the vehicle is ready for pickup.

Vehicle Year

Determines part compatibility, recall eligibility, and labor-time guide lookups. Required because almost every downstream system needs it.

Vehicle Make

The manufacturer — Toyota, Ford, BMW, etc. Used to route the job to a tech with the right training and to check manufacturer-specific TSBs.

Vehicle Model

The specific model — Camry, F-150, 3 Series. Combined with year and make, it uniquely identifies the labor-time and parts lookup for the job.

VIN
Optional

The 17-character vehicle identification number. Optional because not every customer has it handy, but when provided it unlocks exact parts ordering and warranty checks.

Current Mileage

The odometer reading. Used to recommend scheduled maintenance, check warranty coverage, and price the job accurately.

Service needed

A checkbox list of the most common services — oil change, brakes, tires, alignment, A/C, diagnostics, battery, inspection. Customers select one or more to describe the scope.

Issue description

Free-text field where the customer describes the noise, warning light, smell, or behavior they noticed. Often surfaces clues the checklist alone can't capture.

Preferred drop-off date

The date the customer wants to bring the car in. Connect it to a calendar integration to prevent overbooking and auto-confirm appointment slots.

Drop-off or wait preference

A radio button asking whether the customer will wait on-site, drop off and come back, or needs a loaner/shuttle. Helps the service writer plan the bay and the front-desk experience.

Customers fill out year/make/model first because it's the lowest-friction info to recall. Putting it at the top gets them invested before they have to describe the problem.

Match the service checkboxes to your actual service menu — if you don't do alignments, don't show alignment. A short, shop-specific list converts better than a generic mega-list.

Make VIN optional, not required

Most customers don't know their VIN off the top of their head. Requiring it kills mobile submissions. Ask for it, but don't block them.
