What is an online order form?

An online order form is a structured digital form that lets customers place an order, choose options, enter shipping and billing info, and pay — all in one focused flow. Unlike a full ecommerce checkout (which assumes a product catalog, cart, account system, and inventory engine), an order form is purpose-built for situations where the seller has a small set of products, a custom workflow, or a relationship-driven sales process. Customer Name

The buyer's full name as it appears on their card or ID. Required for shipping labels, invoices, and order matching.

Email Address

Primary contact for the order confirmation, shipping updates, tracking number, and post-purchase follow-up.

Phone Number

Backup contact for delivery issues, address verification, and customer service callbacks. Some carriers require it for shipping labels.

Shipping Address

The full street address where the order will be delivered. Required for label generation and shipping cost calculation.

Billing Same as Shipping?

Radio button that, when set to 'no', reveals a second address block for the billing address. Prevents payment failures from address mismatches.

Products

A multi-select checkbox of available SKUs with names, prices, and (optionally) images. Lets the buyer order multiple products in one submission.

Quantity

Number field for how many of each selected product the buyer wants. Pairs with calculation logic to compute the total automatically.

Shipping Method

Radio button for standard, expedited, overnight, or local pickup. Each option can have its own price added to the order total.

Payment Method

Radio button for card, PayPal, ACH, invoice, or pay-on-delivery. Drives the checkout experience based on what the buyer selects.

Show product images on the form

Embed a thumbnail next to each product checkbox. Customers convert at much higher rates when they see exactly what they're ordering — especially for visually driven products like apparel, baked goods, art, and merch.

Keep it to one page on mobile

Unlike intake forms, order forms convert best when they feel like a single focused checkout. Use a one-page layout with clear section headers — customers don't want to step through multiple screens just to buy something.

Calculate the total live

Show a running order total at the bottom of the form that updates as customers add products and choose shipping. Transparency reduces abandonment dramatically — buyers want to know the final price before they reach the payment field.

Default 'billing same as shipping' to yes

Most customers ship to the same address they bill to. Defaulting the radio to 'yes' saves them a step. Only reveal the billing address block if they uncheck it.

Auto-fill the customer's city and state from ZIP

Use Poper's address autocomplete or a ZIP-to-city lookup to fill city and state automatically when the customer enters their ZIP code. Saves time and prevents typos that cause shipping errors.
