About This Template
What is an online order form?
An online order form is a structured digital form that lets customers place an order request, choose options, and enter shipping and billing info in one focused flow. Unlike a full ecommerce checkout (which assumes a product catalog, cart, account system, and inventory engine), an order form is purpose-built for situations where the seller has a small set of products, a custom workflow, or a relationship-driven sales process.
Traditionally these forms lived as PDFs that customers printed, filled out, scanned, and emailed back — or as endless DM threads where the seller had to keep asking 'and what's your shipping address?' six different times. Sellers spent more time chasing details than fulfilling orders, missed shipping deadlines, and lost customers to the friction. The digital order form fixes this: customers fill it out in 90 seconds on their phone, the data is structured for fulfillment, and the seller can spend the day actually packing boxes.
A well-designed online order form does three things at once: capture every detail you need to fulfill the order without back-and-forth, collect payment preference for your billing workflow, and create a structured record you can ship from, account for, and follow up on. This Poper template ships with 11 carefully chosen fields and can route submissions through Google Sheets, HubSpot, Slack, webhooks, Zapier, or Make.
You can use this template as-is for small ecommerce stores, pre-orders and launches, B2B wholesale orders, custom merchandise and apparel, bakery and catering orders, farm-to-table CSA programs, craft sellers on Etsy or Shopify, print-on-demand fulfillment, and service packages — or use the drag-and-drop builder to add product variants, gift messages, delivery dates, custom personalization, and conditional product bundles. The form is fully brandable, works beautifully on mobile, and is free forever on Poper's free plan.

Every Field, Explained
Every field in this form, explained
Each field was chosen to serve a specific fulfillment, billing, or compliance purpose. Here's what each one does — and why it's there.
The buyer's full name as it appears on their card or ID. Required for shipping labels, invoices, and order matching.
Primary contact for the order confirmation, shipping updates from your fulfillment workflow, and post-purchase follow-up.
Backup contact for delivery issues, address verification, and customer service callbacks. Some carriers require it for shipping labels.
The full street address where the order will be delivered. Required for label generation and shipping cost calculation.
Radio button that, when set to 'no', reveals a second address block for the billing address.
A multi-select checkbox of available SKUs with names, prices, and (optionally) images. Lets the buyer order multiple products in one submission.
Number field for how many of each selected product the buyer wants. Helps your team calculate the final order total after review.
Radio button for standard, expedited, overnight, or local pickup. Helps your team prepare the right fulfillment quote.
Radio button for the buyer's preferred method, such as invoice or pay-on-delivery. Helps your team choose the right billing follow-up.
Free-text field for delivery notes, gift wrapping, custom personalization, dietary restrictions, or anything else that doesn't fit a dropdown.
A required checkbox confirming the buyer has read and agreed to the seller's terms, refund policy, and shipping policy.

Best Practices
Online order form best practices
Nine field-tested tips from sellers who have processed thousands of orders through digital forms. Apply what fits your business.
Show product images on the form
Embed a thumbnail next to each product checkbox. Customers convert at much higher rates when they see exactly what they're ordering — especially for visually driven products like apparel, baked goods, art, and merch.
Keep it to one page on mobile
Unlike intake forms, order forms convert best when they feel like a single focused request. Use a one-page layout with clear section headers so customers can submit quickly.
Show pricing before the submit button
Show product and shipping pricing near the submit button. Transparency reduces abandonment because buyers want to understand the likely final cost before submitting.
Default 'billing same as shipping' to yes
Most customers ship to the same address they bill to. Defaulting the radio to 'yes' saves them a step. Only reveal the billing address block if they uncheck it.
Validate shipping addresses early
Ask for the full shipping address and confirm city, state, ZIP, and country in separate fields. Clean address data prevents fulfillment delays.
Set inventory caps for limited drops
For pre-orders, limited editions, or batch production, track quantities in your store or connected sheet and update the form when an item sells out.
Send a branded confirmation email immediately
The moment a customer submits, send a branded confirmation email with the order summary, ship date estimate, and a contact link if they need to make changes. Builds trust and reduces 'where's my order?' support tickets.
Add a coupon or referral code field
A coupon field lets customers share a code without forcing your team to ask later. Validate and apply the discount in your checkout or invoicing workflow.
Tag orders by source
Add a hidden source field that captures whether the order came from your website, Instagram, TikTok, an email blast, or a partner. Use it to figure out which channels actually convert into paid orders.
Offer order pickup or local delivery as cheaper options
Add a 'local pickup' or 'local delivery within X miles' option to your shipping method radio. Customers in your area save on shipping, you save on packaging and carrier fees, and you get a chance to meet your buyers in person — especially valuable for craft sellers, bakeries, and farm-to-table businesses.

Lower Cart Abandonment by 30%
A focused single-page order form dramatically outperforms multi-step ecommerce checkouts for small product catalogs — fewer fields, fewer drop-offs, more completed orders.
Auto-Route by Product, Region, or Method
Conditional logic routes wholesale orders to your sales rep, custom orders to the production team, and same-day delivery requests to your local fulfillment queue.
Send Orders to Your Workflow
Send every order request to Google Sheets, Slack, HubSpot, webhooks, Zapier, or Make so your team can fulfill from the tools it already uses.
Terms Acceptance Included
Every order can include a required acknowledgment of your terms, refund policy, and shipping policy before submission.

Stop running your business out of Instagram DMs and email. Send customers your order form link and watch every detail flow into one structured place.
