About This Template

What is an employee onboarding form?

An employee onboarding form is the structured intake document every new hire fills out before — or on — their first day. It captures the personal information HR needs to set up payroll, the address and emergency contact data IT and benefits need to provision accounts, and the policy acknowledgments your company needs as a defensible legal record. Done well, it's the bridge between an offer letter signed and a productive employee on day one.

Traditionally, onboarding meant a 20-page packet of forms handed to a nervous new hire on Monday morning, filled out by hand under fluorescent lights, and retyped into payroll, HRIS, and IT provisioning tools by an exhausted HR coordinator. It was the worst possible first impression of a company's culture — and the most expensive bottleneck in the entire hiring funnel. Modern onboarding fixes all of this: hires complete the form on their phone the weekend before they start, data flows directly into BambooHR or Gusto, and managers walk into Monday with a fully provisioned employee waiting at their desk.

A well-designed digital employee onboarding form has three jobs: capture accurate personal and tax data, obtain signed acknowledgments of company policies with a legal audit trail, and turn the first day into a welcome — not a paperwork marathon. This Poper template handles all three. It ships with 14 fields HR teams actually use, supports digital signatures and policy checklists out of the box, and integrates with every major HRIS, payroll, and IT provisioning tool via webhook or Zapier.

You can use this template as-is for SaaS startups, agencies, restaurants, retail, manufacturing, healthcare staffing, and remote-first teams — or use the drag-and-drop builder to add country-specific tax forms, role-specific equipment requests, and benefits enrollment. The form is fully brandable, works on mobile, and makes the first impression of your company feel like a warm welcome instead of a cold legal stack.

For You

Why Use This Employee Onboarding Form?

Built and battle-tested by teams who collect real responses every day.

Finished Before Day One

New hires complete every field on their phone the weekend before they start — so day one is for meeting the team, not signing paperwork in a conference room.

Encrypted by Default

Personal data, addresses, and emergency contacts are encrypted in transit and at rest. Role-based access keeps payroll and HR information visible only to the right people.

E-Signature for Policies

New hires type their name to sign off on the employee handbook, code of conduct, and IT policy. Each signature is timestamped and IP-logged for your records.

Captures Emergency Contacts

Emergency contact name, relationship, and phone are required up front — so HR has the file ready before the first shift, not three weeks in.

Policy Acknowledgments Built In

A multi-checkbox section lets new hires acknowledge the handbook, anti-harassment policy, IT acceptable use, and confidentiality clause in one pass.

Warm Welcome, Not Cold Forms

Tone, copy, and brand colors are fully editable — so the form feels like a welcome email from your company, not a stack of legal paperwork.

Every Field, Explained

Every field in this form, explained

Each field was chosen to serve a specific HR, payroll, or compliance purpose. Here's what each one does — and why it's there.

Full Legal Name

The new hire's name as it appears on their government-issued ID. Required for payroll, tax forms, and benefits enrollment.

Preferred Name
Optional

The name the employee would like to be called by colleagues. Used for badges, email aliases, and Slack profiles.

Date of Birth

Used for benefits eligibility, age-restricted role compliance, and as a secondary identifier during payroll setup.

Personal Email

The employee's personal email address for sending login credentials, the welcome packet, and any communications before their work email is provisioned.

Phone Number

Backup contact for first-day coordination, urgent notifications, and same-day reminders if onboarding tasks are still incomplete.

Home Address

Required for payroll tax withholding, benefits enrollment, equipment shipping for remote hires, and W-2 mailing at year end.

Start Date

The employee's official first day. Drives payroll proration, benefits effective dates, and IT account activation timing.

Role / Position

The job title for org charts, internal directories, and routing the right policies, equipment, and onboarding content.

Reporting Manager

The direct manager's name. Used to grant the manager visibility into the onboarding submission and trigger a welcome notification.

Emergency Contact

Name, relationship, and phone of the person to contact in a workplace emergency. Required by most labor regulations and HR policies.

T-Shirt Size
Optional

For ordering company swag and welcome kits before the first day. Optional, but a nice touch that makes the welcome feel intentional.

Dietary Preferences
Optional

Captures allergies, vegetarian, vegan, kosher, and halal preferences for team lunches and catered events.

Policies Checklist

Multi-checkbox acknowledgment of the employee handbook, anti-harassment policy, IT acceptable use, and code of conduct. Each checkbox is timestamped.

Digital Signature

A typed full-name signature that, together with the timestamp and IP address, forms a legally defensible e-signature record for the entire submission.

Best Practices

Employee onboarding form best practices

Nine field-tested tips from HR teams that have onboarded thousands of employees. Apply what fits your company stage and culture.

1

Send the link the day the offer is signed

Don't wait until the day before. Trigger the onboarding link the moment your ATS marks the offer accepted. New hires are most engaged in the 48 hours after signing — capture that energy.

2

Break the form into named steps

A 14-field one-page form looks intimidating. Group fields into clearly named steps: 'About You', 'Contact & Address', 'Role & Manager', 'Policies & Signature'. Completion rates jump 30–40% with step layouts.

3

Use conditional logic for international hires

Add a 'country of work' dropdown and reveal country-specific tax forms, contracts, and banking fields. One template covers every region without forking the form.

4

Pre-fill known data from your ATS

If the candidate already provided their name, email, and start date in your ATS, pre-fill those fields via URL parameters. The new hire only fills out what's actually new — most finish in under five minutes.

5

Make truly optional fields optional

T-shirt size, dietary preferences, and pronouns should never be required. Marking everything required slows hires down and signals a rigid culture before they even start.

6

Restrict access by role

Direct deposit, SSN, and tax data should be visible only to payroll. Use role-based permissions so managers see basic info and HR sees the full record.

7

Email a welcome PDF after submission

Auto-generate a PDF copy of the completed onboarding and email it to both the hire and their manager. Include a welcome letter, first-week schedule, and Slack invite link in the same email.

8

Trigger IT provisioning automatically

When the form is submitted, fire a webhook to your IT provisioning tool (Okta, Rippling IT, Bettercloud) to create accounts in Slack, email, GitHub, and the rest of your stack. The hire walks in with everything ready.

9

Reuse the form for ongoing employee data updates

Send a stripped-down version annually so employees can update their address, emergency contact, and dietary preferences. Keeps your HRIS clean without an HR coordinator chasing 200 people on email.

For Teams & Businesses

Built for Professional Use

Import this form into Poper, brand it, and embed it anywhere. Responses flow straight into your tools.

Cut First-Day Onboarding by 70%

Companies that move onboarding paperwork online before day one save 60–90 minutes of HR time per hire and free up the manager to focus on welcome and training.

Conditional Logic by Role and Location

Show state-specific tax forms, country-specific contracts, and role-specific equipment requests automatically. One form, every employee type.

Push Into BambooHR, Gusto, Rippling

Send completed submissions straight to your HRIS, payroll, ATS, and IT provisioning tools via webhooks, Zapier, or 96+ direct integrations.

Audit-Ready Records

Every submission is timestamped, IP-logged, and exportable as a PDF with the digital signature attached — giving you a defensible record for every new hire.

Brand It Like Your Company

Swap in your logo, brand colors, and welcome copy. New hires see your culture from the very first form they touch — not a generic builder.

Automated Reminders Until Done

Trigger an email or SMS the moment a hire is missing a field, and remind them again 24 hours before their start date if anything is still incomplete.
